Autologous Latissimus Dorsi Breast Reconstruction

Abstract

Breast cancer surgical treatment results in important damage to women. The damage is to both the body and the mind. Breast reconstruction belongs to the treatment of breast cancer. An increasing number of patients benefit from immediate or delayed reconstruction. Breast reconstruction is a difficult challenge which can help women in their own reconstruction. Among many breast reconstruction surgical procedures, latissimus dorsi flap reconstruction is our technique of choice, and provides the best results in our experience. We describe an anatomical study of the latissimus dorsi flap, and then the surgical procedures. We also describe indications for and limits of the technique. Finally, we present our results with long-term follow-up. A second stage is lipomodeling of the breast 2 or 3 months after latissimus dorsi flap reconstruction. It provides shape to and projection of the breast. Fat grafting is done into the latissimus dorsi flap, which can be considered as a matrix recipient for fat grafting. In conclusion and in our experience, the latissimus dorsi flap is the first step in and first choice for autologous breast reconstruction. It is a safe and reliable technique. It provides an excellent recipient for fat grafting 2 or 3 months later to produce a natural autologous breast.
