Verslaving behoort samen met angst en depressie tot de top drie van de meest voorkomende psychiatrische stoornissen in Nederland. Alcohol- en drugsgebruik kan leiden tot gezondheidsproblemen, sociale en financiële problemen, overlast en criminaliteit. Slechts een klein deel van de verslaafde populatie meldt zich voor hulp bij de verslavingszorg. Dan blijkt vaak dat stoppen of minderen niet eenvoudig is en dat terugval in verslavingsgedrag eerder regel is dan uitzondering. In toenemende mate wordt verslaving beschouwd als een langdurende, chronische ziekte, waarbij de belonende werking van alcohol en drugs op de hersenen een centrale rol speelt. Juist deze ‘beloningsaspecten’ staan centraal in de Community Reinforcement Approach (CRA). Deze aanpak is voor Nederlandse begrippen een relatief nieuwe benadering die zich richt op het ontwikkelen van een alternatieve leefstijl die meer belonend is dan middelengebruik. CRA is een cognitief-gedragsmatige behandeling die zich focust op gedragsverandering bij mensen die afhankelijk zijn van middelen. Het is gebaseerd op de aanname dat bekrachtigers uit de sociale omgeving van grote invloed zijn en bepalen of het gebruik van alcohol en/of drugs wordt voortgezet. CRA richt zich dan ook primair op bekrachtigers in de fysieke omgeving, zoals het gezin, woonomgeving, vrienden, vrijetijdsbesteding, werk en dergelijke. CRA erkent het belang van omgevingsfactoren bij het aan- of ontmoedigen van middelengebruik en probeert deze factoren zo te beïnvloeden dat een leefstijl zonder drugs en/of alcohol als waardevoller wordt beleefd. Daarnaast combineert CRA op een creatieve wijze met farmacotherapie, om de bekrachtigende werking van psychotrope stoffen te reduceren. Er wordt gebruikgemaakt van een scala aan interventies. CRA omvat diagnostiek en functieanalyse; farmacotherapie; verbeteren van therapietrouw; monitoring van gedrag en middelengebruik (bijvoorbeeld urine en BAC2); relatietherapie; sociaal-netwerkbegeleiding; oriëntatie op werk, opleiding en vrijetijdsbesteding; probleemoplossende vaardigheden; sociale vaardigheidstrainingen en terugvalpreventie. Het doel van CRA is echter veel meer dan het aanbieden van een verzameling van therapeutische interventies. Het is een geïntegreerde aanpak om verslaafden te assisteren bij het veranderen van schadelijk gedrag.
