3 Zelfmanagement voor chronisch zieken; een behandeloptie voor fysiotherapeuten?

Samenvatting

In dit hoofdstuk wordt beschreven wat onder zelfmanagement wordt verstaan en waarom zelfmanagement belangrijk is voor chronisch zieke ouderen. Er wordt uitgebreid ingegaan op een interventie die geschikt is voor ouderen met comorbiditeit, namelijk de Chronic Disease Self-Management Program. Verder wordt ingegaan op de rol van zelfmanagement in de fysiotherapie en wat de meerwaarde hiervan kan zijn bij patiƫnten met chronische aandoeningen.
