Het eruptiepotentieel van ingesloten derde molaren in de mandibula

Samenvatting

Derde molaren hebben vaak te weinig plaats om in een normale positie door te breken in het meest distale deel van de mond. Recent radiologisch onderzoek toont aan dat slechts 2 % van de derde molaren in een (orthodontisch behandelde) mandibula met volledige dentitie kan doorbreken. Partieel of volledig ingesloten derde molaren kunnen uiteenlopende problemen veroorzaken, waardoor er vaak voor wordt gekozen om deze molaren (preventief) te verwijderen. Een ongunstige doorbraakrichting van de derde molaren kan gemakkelijk gevisualiseerd worden met een panoramische röntgenopname. Daarbij kan niet alleen het eruptiepotentieel van de derde molaar beoordeeld worden, maar ook de relatie met het canalis mandibularis. Bij vier van de vijf mesiaal geïnclineerde ingesloten derde molaren bevindt de wortelapex zich op minder dan 2 mm van nervus alveolaris inferior. Bij vermoeden van een relatie, op basis van de panoramische röntgenopname, vergroot de kans op zenuwschade bij de chirurgische verwijdering van de verstandskies. Het is dan belangrijk het driedimensionale verloop van de nervus alveolaris inferior en de relatie met de wortels van de derde molaren en andere omringende anatomische structuren goed in beeld te brengen door middel van aanvullende CBCT-beeldvorming.
