Zusammenfassung
Hochenergetische Beckenverletzungen stellen aufgrund der Gefahr für massive Blutungen lebensbedrohliche Verletzungen dar, die einer schnellen mechanischen Stabilisierung sowie Blutungskontrolle bedürfen. Eine mechanische Stabilisierung kann durch verschiedene Maßnahmen vom Beckenbinder, dem externen Fixateur bis hin zur Beckenzwinge erreicht werden, wobei all diese Maßnahmen spezifische Vor- und Nachteile aufweisen. Eine Blutungskontrolle kann häufig bereits durch die mechanische Stabilisierung erreicht werden, umfasst regelhaft aber auch das „pelvic packing“ und zunehmend auch die interventionell radiologische Gefäßembolisation. Die endovaskuläre Ballonokklusion der Aorta kann als Ultima Ratio bei unkontrollierbaren Blutungen in Betracht gezogen werden. Die definitive Versorgung sollte nach Abklingen der posttraumatischen Inflammation frühestens ab dem fünften Tag nach Trauma erfolgen.
